How do I submit a case?

There are two ways to submit a case. The first is to fill out the ‘request supplies’ form on our send a case tab. The second is to download the printable versions of the prescription forms and submit them with your order. If you have a special request or require further information, you can call us at (845) 831 2216 or submit a form on the contact us page.

Do You Offer Additional Services?

Hudson Valley Dental Laboratory is a full-service dental laboratory that offers a variety of services ranging from the planning, design and fabrication of artificial replacements, ceramics, implants and crowns and bridges.
