How To Send in a Case With 6 Easy Steps:

    Complete the prescription form for your case. Hudson Valley Dental can send the forms free of charge, or digitized versions of our dental lab RX templates can be downloaded directly from our website. click to download

    Place study models, impressions or bites in the inner pocket of the transparent pouch.

  • 3

    Place the completed dental lab prescription printing form in the outer pocket of the pouch.

  • 4

    Once the pouch is secured, place it in a bubble wrap bag and then put the package in the box provided.

    Secure the dental lab slip shipping label to the top of the box.

    If using Federal Express labeling, call FedEx at 1 800 463 3339. Select 1, then 2, then 2 again and enter your shipment number to track the parcel. You can schedule a pickup online by Clicking Here.
